Before Your Visit

We want to make your office visit a hassle-free and pleasant experience. For new patients, please complete the New Patient Registration form and print it out for your office visit with Dr. Kethu. This will simplify the check-in process. Also, please make sure to bring the following information:

Insurance card
Driver’s license
List of current medications
If you have any relevant old records, please bring them. If you were seen in the ER or by another physician recently, please let our office staff know so that we can try to get the records prior to your visit.

Plan to arrive 15 minutes prior to your appointment and have a means of covering your financial portion as determined by your insurance plan. If you will be paying by credit card, we accept Visa, MasterCard, AMEX and Discover.
